Understanding what is really going on

What tends to be behind it
No two presentations are the same, which is why the assessment matters more than the label.
Mechanical and postural
Long hours seated, repeated lifting or a sudden awkward movement can overload structures in the spine and surrounding muscles.
Disc related
Bulges and disc irritation can refer pain into the buttock and leg. These respond well to the right loading, and badly to the wrong kind.
Joint and soft tissue
Facet joints, the sacroiliac joint and deep spinal muscles can all generate genuine, localised pain.
Deconditioning and load
When the back has not been loaded for a while, ordinary demands start to feel like too much. That is a capacity problem, not damage.
