What tends to be behind it

No two presentations are the same, which is why the assessment matters more than the label.

Disc irritation

The most common driver. A disc bulge can chemically or mechanically irritate the nerve root as it exits the spine.

Lateral canal narrowing

Reduced space where the nerve travels, more common with age, often worse standing and walking, easier when seated.

Gluteal and deep hip tissue

The sciatic nerve passes close to deep hip muscles. Tension and irritation here can mimic true nerve root pain.

Sensitisation

When symptoms have been present a while, the nerve itself becomes more reactive and less tolerant of ordinary load.

Sciatica: common questions

Is sciatica serious?
In most cases it settles well with the right management. There are a small number of red flag signs, such as loss of bladder or bowel control or significant numbness in the saddle area, that need urgent medical attention. These are screened for at every assessment.
How long does sciatica last?
Many cases settle substantially within six to twelve weeks. Symptoms that have persisted longer usually still respond, they simply need a more graded approach.
Will I need surgery?
The large majority of people never do. Surgery is generally considered only when symptoms fail to respond over time or where there is progressive neurological loss.
Can I exercise with sciatica?
Usually yes, and often you should. The key is the type and dose. Part of the assessment is establishing exactly what your nerve currently tolerates.
