Shoulder Pain Treatment in Rainham & Medway

The shoulder trades stability for movement, which is what makes it so capable and so easily irritated.

The starting point

Understanding what is really going on

The shoulder trades stability for movement, which is what makes it so capable and so easily irritated. Most shoulder pain is not a tear that needs fixing. It is a tissue that has been asked to do more than it is currently ready for, and it responds well to being rebuilt properly.

“Previous physio felt very basic, exercises and that was it. Josh takes a completely different approach. He considers your mental health, lifestyle and how everything connects.”

Common drivers

What tends to be behind it

No two presentations are the same, which is why the assessment matters more than the label.

Rotator cuff related

The most common source. Usually a loading and capacity problem rather than a structural failure, and highly responsive to graded strengthening.

Impingement and bursitis

Pain on reaching overhead or behind. Typically involves how the shoulder blade and cuff coordinate under load.

Frozen shoulder

A distinct condition with recognisable stages. Getting the stage right matters enormously, as the wrong treatment at the wrong point makes it worse.

Neck referred

A meaningful proportion of shoulder pain originates in the neck, which is why both are always assessed.

How we work

What treatment actually looks like

Work out the source

Assessment of the shoulder, shoulder blade, thoracic spine and neck to identify what is actually generating symptoms.

Settle it down

Manual therapy, load management and clear advice on what to modify in the short term.

Rebuild the cuff

Progressive strengthening of the rotator cuff and surrounding musculature. This is the part that produces lasting change.

Restore full function

Overhead, behind the back, in the gym or at work, until the shoulder is no longer something you think about.

FAQs

Shoulder Pain: common questions

Is my shoulder pain a rotator cuff tear?
Many rotator cuff changes are found in people with no pain at all, so a tear on a scan does not automatically explain symptoms. Most cuff related pain improves substantially with structured strengthening.
How long does frozen shoulder take?
It follows a recognised course that can run twelve to eighteen months untreated. The right treatment at the right stage improves both comfort and range considerably along the way.
Why does it hurt more at night?
Shoulder pain frequently worsens at night due to sleeping position and reduced distraction. It is common and it is addressable.
Will I need an injection?
Sometimes an injection helps create a window to rehabilitate. It works best alongside a loading programme rather than instead of one.
